As a courtesy to you, we verify insurance benefits and eligibility 2 days prior to your visit. Using your personal insurance benefit information, we will calculate an estimate of your patient responsibility. Any applicable copayment, coinsurance and unmet deductible will be collected at the time of service. If payment is not arranged, we may ask you to reschedule your appointment.
